Le kyste mandibulaire vestibulaire surinfecté est une lésion peu commune, associée aux molaires mandibulaires chez l’enfant, au moment de leur éruption. Le cas décrit concerne un enfant avec des kystes bilatéraux en relation avec 36 et 46 retenues. Les dents étaient vitales et indolores mais leur éruption était bloquée. Le CT-scan montrait des images kystiques en position vestibulaire. Il a été décidé d’extraire les dents incriminées et d’énucléer les kystes. Un examen anatomopathologique a confirmé la nature kystique des lésions. Le diagnostic de kyste mandibulaire vestibulaire surinfecté a pu être posé bien que cette dénomination soit peu adaptée en raison de l’absence d’une réelle infection.
Abstract
The mandibular infected buccal cyst is an uncommon lesion associated with the permanent mandibular first or second molar in children just prior to tooth eruption. The WHO includes this lesion under the category of paradental cyst. We describe the case of a patient with bilateral mandibular cyst affecting the first molars. The 9-years-old boy presented with no pain on the impacted teeth. The vitality tests were positive. Computed tomography showed a well defined bilateral ovoid radiolucencies at the apices of the lower left and right first molars. The treatment was to enucleate the cysts and extract both teeth. The histopathologic diagnosis of both lesions was a paradental cyst. The WHO suggests the term of mandibular infected buccal cyst (MIBC). The aim of this case report was to familiarize dentists and oral surgeons with the MIBC, and to discuss the different treatments that are suggested in the literature.
