Introduction : La projection d’une troisième molaire mandibulaire dans l’espace ptérygo-mandibulaire est une complication exceptionnelle, surtout sans pathologie associée. Observation : Le cas d’une patiente âgée de 19 ans, sans antécédents médicaux particuliers, présentant la dent 48 au sein de l’espace masticateur suite à une avulsion, a été décrit. Discussion : Cet événement survient surtout lorsque la dent est associée à un kyste, ou lorsque le décollement en direction linguale est important et que la dent glisse dans cet espace libre. Conclusion : Il est nécessaire de récupérer rapidement la dent afin d’éviter une infection ou sa migration plus profondément dans les tissus, ce qui entraînerait un geste chirurgical plus invasif.
Abstract
Introduction: Accidental displacement of the mandibular third molar into the pterygomandibular space is a rare surgical complication, especially when there is no underlying pathology. Observation: The case of a 19-year-old patient with no general health problems, presented a lower right third molar in the masticator space after its extraction, is reported. Discussion: The ectopic position is more often related to the presence of a cyst, or when the lingual subperiostal dissection is too important leading to a slip of the tooth into that free space. Conclusion : it is necessary to quickly retrieve it in order to avoid an infection or a migration deeper into the soft tissues, which would then require a more invasive surgical procedure.
